What this form is for
This form is used to process in-kind donations to Past Lives Makerspace (PLM) with a stated fair market value under $5,000. An “in-kind” donation is a non-cash gift — physical goods rather than money. For PLM, that typically means equipment, machinery, tools, materials, supplies, or consumables. (Cash donations are processed separately.) If your donation’s stated value will be $5,000 or more, please reach out before filling this in — IRS rules require a qualified appraisal at that level, and the process is different.

Past Lives Makerspace is a 501(c)(3)-pending nonprofit. Until our 501(c)(3) determination is granted by the IRS, all tax-deductible contributions are processed through our fiscal sponsor, Oregon Harbor of Hope — a registered 501(c)(3). After we receive your donation, Oregon Harbor of Hope will issue your tax-acknowledgment letter on their letterhead, with their EIN, for your records.
